Does this satisfy OSHA's requirement?
OSHA's Respiratory Protection Standard (29 CFR 1910.134) requires a medical evaluation before an employee can be fit tested or wear a respirator on the job. Our questionnaire is based on OSHA's mandatory medical questionnaire (Appendix C), and every response is reviewed by a licensed clinician — the PLHCP the standard requires.
How long does it take?
The questionnaire takes most people just a few minutes, and you'll typically receive your clearance within 24 hours.
Who sees my health information?
Only the licensed clinician reviewing your evaluation. Your employer receives only your clearance determination — never your medical answers.
What happens if my answers need a closer look?
We'll contact you directly to schedule a follow-up with the reviewing clinician — often resolved with a short call.
Do I need to repeat this every year?
OSHA doesn't require the medical evaluation on a fixed schedule. A new evaluation is needed if your health changes, your workplace conditions change, or your employer requires one.
